About the role
This is a senior clinical leadership role that provides advanced social work expertise, consultancy and complex case management for patients with disabilities, including current and prospective NDIS participants, patients with significant psychosocial needs, and individuals requiring transition to residential aged care. Working across the continuum of care, you will play a critical role in supporting safe, timely and patient-centred care while helping to address complex discharge planning and length-of-stay challenges.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- In accordance with the Australian Association of Social Workers (AASW) Code of Ethics and AASW Professional Practice Standards, deliver specialised social work clinical and consultancy service including functioning as a resource expert with advanced level clinical knowledge for patients with physical, neurological, psychiatric, developmental cognitive and sensory disabilities.
- Provide independent social work clinical services of a highly complex and varied nature where principles and procedures may require constant adaptation or modification to address clinical requirements and legal standards to enable individuals with disabilities to live independently.
- Work collaboratively with and provide high-level clinical advice and direction to social work practitioners and a range of health professionals, supervisors,
relevant service managers and other stakeholders regarding social work and interprofessional management of NDIS participants and service delivery including prioritisation and changes in clinical practice, equipment and technological advances.
- Demonstrated advanced clinical reasoning and ability to act as a patient advocate, including escalation to Senior Leadership Team (via appropriate channels) around patient flow concerns.
- Provide professional leadership in the development of clinical practices, procedures and protocols that support the equitable access and health care of individuals with disabilities, engaging Hospital and Health Service and state-wide cross-discipline referencing where relevant.
