About the Role
We are seeking Full-Time Care Coordinator (Regional Area Coordinator) to manage day-to-day coordination of participant services across Greater Melbourne.
You will act as the central point of contact between participants, families, support workers, Support Coordinators, allied health professionals and internal teams, ensuring services are well planned, consistently delivered and compliant with NDIS Practice Standards.
This is a field-based coordination role that combines:
- Regular participant visits
- Service onboarding and compliance
- Rostering stability
- Incident management
- Stakeholder relationship building
- Operational coordination
Key Responsibilities
Participant Intake & Onboarding
- Conduct intake discussions to understand participant goals, risks and preferences
- Prepare service agreements and complete compliant onboarding documentation
- Set up structured and reliable support rosters
Participant Visits & Service Monitoring
- Conduct regular home and community visits for the participants
- Monitor service quality and participant satisfaction
- Collect feedback and address concerns proactively
Workforce Coordination & Rostering
- Match suitable support workers to participant needs
- Maintain stable rosters and manage short-notice changes
- Communicate staffing requirements to the internal recruitment team
Quality, Documentation & Compliance
- Ensure accurate case notes,
incident reports and feedback records
- Uphold NDIS Practice Standards, safeguarding and privacy requirements
- Ensure participant files remain compliant and up to date
Incident Management
- Respond to incidents occurring during support shifts
- Coordinate follow-ups, risk mitigation and communication with stakeholders
Stakeholder Relationship Management
- Maintain strong relationships with participants, families, Support Coordinators and clinicians
- Build new referral networks and community partnerships
- Provide service updates and resolve issues efficiently
Training & Support
- Guide support workers on service expectations, documentation and person-centred practices
- Support continuous improvement in service delivery
Continuous Improvement
- Track participant outcomes
- Suggest improvements to processes and service quality
What You’ll Bring
- Qualification in Community Services, Social Work, Mental Health or related field (or relevant experience)
- Experience in NDIS coordination, rostering or client relationship roles
- Familiarity with CRM systems such as MYP, Brevity, ShiftCare or similar
- Solid communication, empathy and problem-solving skills
- Ability to manage competing priorities in a fast-paced environment
- Good administrative and Microsoft Office skills
- Current First Aid & CPR (or willingness to obtain)
- NDIS Worker Screening Check and Working With Children Check (or willingness to obtain)
- Valid Driver’s Licence and reliable vehicle

Location & Hours
Full-time permanent role based in Melbourne with travel across Greater Melbourne as required.
