Supporting Patients navigating Residential Aged Care

This role will provide specialised guidance to patients and families in navigating their residential aged care journey. Be a part of a team that continues to build relationships with external agencies with a focus on advocacy and meeting patients ongoing needs; whilst working alongside multi-disciplinary clinicians and closely with social work team to coordinate safe and timely discharges within the hospital setting.

About the role:

Reporting to the Director of Social Work, this role will be involved in facilitating the assessment, listing and placement of those admitted under the Division of Rehabilitation, Aged Care and Palliative Care (Division of RAP) who have been identified, by clinical staff, as unable to discharge to their previous residence.

Duties for the role will include:

- Provide, in consultation with Division of RAP Social Workers, advice and assistance to patients and families of patients regarding assessment processes for residential aged care facilities and on the availability and access to these facilities.
- Assist patients, families and carers with application forms and other documentation as required.
- Liaise with Residential Aged Care facilities and other aged care and community service and accommodation providers to facilitate access to these facilities and services by patients of the Division of RAP.
- Maintain statistical and management information for planning, monitoring and reporting purposes, with emphasis on key performance indicators for the Aged Care Assessment Team and placement process, and on patient transfers to Residential Aged Care Facilities and Transitional Care places.
- Contributing to the development of an integrated team approach and culture which is highly responsive to the needs of our consumers.

About you:

- Variable working locations may be required, including Noarlunga Hospital, Flinders Medical Centre and Repat Health Precinct.
- Ability to relate to a wide range of people, including patients, families, clerical staff, clinical staff, Aged Care Assessment Teams and Directors, Supervisors and Managers of Residential Aged Care Facilities and Supported Residential Accommodation facilities.
- Ability to empathise with patients, families and carers.
- Good oral and written communications skills, ability to negotiate, and to manage conflict.
- Ability to co-ordinate and prioritise a number of tasks simultaneously to meet deadlines.
- Ability to work effectively both independently and as part of a team.
- Ability to analyse statistical data and develop options and make recommendations on this basis.
- Proven experience in basic computing skills, including email and word processing.
