About the Program:
This service provides multidisciplinary, community-based support to individuals experiencing severe and complex mental health, psychosocial, behavioural, and care coordination needs across rural and regional communities. The program delivers intensive, recovery-oriented support for patients who may experience barriers accessing traditional mental health services due to geographic isolation, complexity of presentation, psychosocial vulnerability, service fragmentation, or co-occurring challenges. Based in a GP practice, the service model includes outreach, multidisciplinary collaboration, care coordination, and integrated care with local health, social, and community services to improve outcomes for individuals with complex support needs.

Position Purpose:
As an allied health clinician within a multidisciplinary team, you will provide assessment, therapeutic intervention, and coordinated support for individuals experiencing severe and complex mental health presentations in rural and regional communities. You will manage a complex and varied caseload, delivering discipline- specific care while working collaboratively within a general practice environment.

Key Responsibilities:
Clinical Assessment and Intervention

Provide assessment and intervention for individuals with severe and complex mental health and psychosocial presentations.

Undertake comprehensive screening, assessment, care planning, therapeutic intervention, and care coordination.

Identify clinical deterioration, risk escalation, safeguarding concerns, and urgent support requirements.

Support assertive engagement approaches to improve continuity of care and service access.

Support patients who may be at elevated risk, vulnerable to disengagement, or experiencing significant social complexity.





Escalate or de-escalate care appropriately across stepped-care or acute services.

Multidisciplinary & Collaborative Practice

Participate in multidisciplinary case conferencing, risk management, and coordinated response planning.

Develop collaborative relationships with hospitals, primary care, community services, crisis services, and external agencies.

Contribute to integrated care planning for patients with co-occurring mental health, physical health, behavioural, housing, and social support needs.

Outreach & Clinic Hub Facilitation

Deliver flexible outreach support including community-based appointments and home visits.

Attend and facilitate appointments at regional clinic hubs to ensure equitable access for patients who cannot attend independently.

Provide care within non-traditional environments to support engagement and continuity of care.

Data Collection, Documentation & Reporting

Maintain high-quality progress notes, documentation, and reporting in line with professional and program requirements including the primary outcome measures including K10 and MHQoL questionnaires, contributing to the PMHC-MDS.

Gather relevant information for psychiatrist review and multidisciplinary planning.

Participate in quality improvement, evaluation, and service development activities.

Engage in professional supervision in line with discipline and organisational requirements.

Maintain professional accountability and adhere to trauma-informed, recovery-oriented, culturally safe, and person-centred practice frameworks.

Essential Requirements:
Registration Requirements

Mental Health Social Worker: Accredited Mental Health Social Worker (AMHSW) credential with the Australian Association of Social Workers.

Experience & Capability

Demonstrated experience working with individuals experiencing severe and complex mental health and psychosocial needs.

Advanced clinical skills and independent judgement in managing complex caseloads.

Experience managing presentations involving risk,



service coordination, and multi-stakeholder involvement.

Strong understanding of trauma-informed, recovery-oriented, culturally safe, and person-centred practice.

Ability to work autonomously while managing competing clinical priorities across rural settings.

High-level communication, collaboration, and stakeholder engagement skills.

Demonstrated cultural competence and responsiveness to diverse populations.

Ability to rapidly adapt to working within a GP practice and engage with the whole-of-practice care team.

Current driver's licence and willingness to undertake outreach travel and home visits.

Role Complexity:
This role requires management of a high-complexity clinical caseload within a rural outreach environment. Clinicians must exercise substantial skilled judgement, independent decision-making, and advanced care coordination capability.
The role routinely involves engagement with individuals experiencing significant psychosocial complexity, co-occurring conditions, elevated risk factors, service access barriers, and fragmented support systems. Clinicians are expected to operate with a high degree of autonomy while contributing to integrated multidisciplinary care planning and system navigation.

Highly Desirable:

Postgraduate qualifications in mental health.

Experience working in rural or remote communities.

Experience supporting patients with co-occurring mental health and psychosocial complexity.

Experience in community-based, outreach, or multidisciplinary mental health service delivery.

Experience delivering assertive outreach or intensive community-based support, particularly with patients who may disengage from traditional service models.

Understanding of stepped-care models including the IAR Decision Support Tool.
