The Salvation Army Australia is seeking a dedicated Case Manager for the Homeless Outreach Program at Nunawading, VIC.
The permanent part time role offers 22.8 hours per week, reporting to the Team Leader.The role focuses on outreach, advocacy and holistic support to people experiencing homelessness to improve wellbeing and housing stability.
You will provide tailored plans, coordinate housing and health services, and maintain detailed client records while working within the SCHADS Award Level
