As a Care Coordinator, you'll use your clinical expertise to support members through care planning, education, discharge support and recovery management.
You'll work closely with hospitals, GPs and allied health providers to reduce avoidable admissions and improve health outcomes.
Key responsibilities: Develop and manage personalised care planProvide clinical advice, coaching and educationCoordinate care across hospitals, primary care and community servicesSupport recovery pathways, medication, pain and falls managementContribute to quality, accreditation and continuous improvement About you: Must have relevant clinical experienceMinimum 5 years' relevant experienceStrong knowledge of the public and private health systemsConfident, cooperative and able to work autonomously Essential Qualifications:



Bachelor of Nursing (or equivalent qualification leading to RN registration).
Current AHPRA registration as a Registered Nurse.
