We're seeking an experienced and compassionate Registered Nurse to join our Home Care team, supporting the delivery of high-quality clinical care to clients living independently in the community.
Reporting to the Customer Care Manager, you'll provide clinical coordination, case management and nursing support to Home Care Package clients. This is a varied role that combines clinical care, care planning and stakeholder engagement while working closely with clients, families and multidisciplinary teams.
Day to day
- Deliver person-centred nursing care to clients in their homes.
- Conduct assessments and develop and review individualised care plans.
- Monitor client health and provide clinical advice and support.
- Maintain accurate clinical documentation and records.
- Collaborate with clients, families, GPs and allied health professionals.
- Ensure compliance with Aged Care Quality Standards and organisational policies.
- Contribute to continuous improvement initiatives and best practice care.

You're a compassionate RN Nurse who enjoys building relationships and supporting older Australians to live independently.
You’ll have current AHPRA registration and ideally 5+ years' nursing experience within aged care, community or hospital settings. You’ll be confident in clinical assessments and care planning, with a strong understanding of aged care standards. As you’ll be visiting clients in their homes, a current driver's licence and access to a reliable, fully insured vehicle are essential.
