Home Stretch WA Transition Coach – Case Manager

Permanent, full‑time (38 hours per week). Salary: $94,472.36 + super + NFP salary packaging. Location: Goomburrup (Bunbury), WA 6230.

Mission Australia is committed to ending homelessness and supporting those in need. As a Home Stretch Transition Coach, you will provide individualized, professional support and case coordination to young people aged 0–21, helping them transition from out‑of‑home care to independence.

Key Responsibilities

- Develop a young person’s natural networks and assist them to build and maintain connections to culture, country, family, and community supports.
- Offer and broker access to a safety net of financial support and resources through the Home Stretch WA service offer.
- Provide tailored support services using a culturally safe and strengths‑based approach and case coordination in line with the Home Stretch WA Transition Coach Practice Principles.
- Work collaboratively with relevant stakeholders including the Department of Communities Child Protection staff, foster families, carers, the young person’s kin, community and other significant parties.
- Ensure the right to self‑determination drives your practice while promoting the wellbeing of young people.

Requirements for Success

- A Degree or Diploma in Social Welfare/Social Science or a similar field and a minimum of two years’ experience in a comparable role.




- Comprehensive understanding of the barriers and issues affecting young people who have been in state care, including those disengaged from education, training and employment.
- Ability to work in a culturally safe, trauma‑informed manner with strong knowledge of Aboriginal culture.
- Proven experience in providing case management, assessment, counselling and crisis intervention for vulnerable young people with complex needs.
- Strong interpersonal and networking skills, able to build effective relationships with Aboriginal communities and services.
- Excellent written and verbal communication skills and high‑level computer literacy.
- Ability to work flexibly in a solutions‑focused manner, independently and as part of a diverse team.
- WA CPFS Child Protection Screening Checks.
- Full driver licence with no restrictions.
- Will undertake a national police history check, qualifications and referee checks, and obtain a clearance to work with children/vulnerable people.
