Job Description

The opportunity

IFYS hasanexciting chance foraTransition Case Managerto join theBeyond Expectations team.

Operating from a client-centred and strengths-based framework, you will work with young people aged 15–19 years who are or have been involved with the Department of Child Safety. The role focuses on supporting young people to identify and achieve their goals, reduce vulnerability, and increase their independence, wellbeing, and connection to the community.

A key focus of the role is supporting education, training, and employment outcomes through the Beyond Expectations Program.

- Fixed-term, part-time position with initial contract expiring 18/06/2027
- Minimum60hours perfortnight, working four days Monday-Friday, hours between 8:30am and 4:30pm
- Salary base $88,099per annum pro rata + superannuation– IFYS is committed to gender pay equity

Key responsibilities

- Receive and assess referrals, determining eligibility and priority of response
- Support young people to identify their needs and goals, through the development and implementation of individualized goal plans including the management of brokerage funds
- Support young people to obtain job and training readiness skills and access pathways to achieve education, training and employment goals
- Connect with local business and employers to identify work experience and employment opportunities
- Identify and assess risk level for young people and respond appropriately, engaging other support services when required
- The timely and accurate completion of administrative duties, case notes, case plans, data collection,



financial management, timesheets and incident reports in line with legislative and contractual requirements
- Work collaboratively with key stakeholders including Youth Justice and Community Corrections
- Advocate on behalf of individuals to ensure that services accessed and provided are relevant, flexible and accessible.
- Contribute positively to the development of resources, work procedures and processes, funding submissions and other projects

Requirements for the role:

- Professional experience working with young people in a therapeutic capacity
- Diploma or Tertiary qualification in a relevant area of study will be considered favourably
- Demonstrated knowledge of strengths-based and client-centred practice
- Demonstrated strong cultural knowledge and understanding of First Nations communities, with the ability to engage and support young people in a culturally sensitive and respectful manner

Must hold or be willing to obtain if successful

- Current Queensland driver's license with no restrictions, own reliable transport and a clear traffic history
- QLD Working with Children Check (Paid Blue Card)
- As part of our recruitment process, we do a number of history screenings including a Licensed Care Suitability check (LCS2) and a National Crime Check
- Ability to provide two references from recent employers (including your current line manager)
