NCN Health is seeking an Activity Assistant to join our Community Health and Wellbeing team.

The Activity Assistant supports the Aged Care and Disability team to work within the planned activity group programs by assisting with:

- Supporting clients to participate in group-based activities and outings
- Assisting with planning and implementation of individual and group activities
- Supporting the transportation arrangements for clients within the groups, acting as driver or attendant when required;
- Maintenance of accurate and concise records of client attendance, daily program provision and client needs
- Supporting clients to develop a Coordinated Care Plan in accordance with the client’s goals and wellness and reablement principles.

About You
- Vaccination against seasonal Influenza (mandatory)
- Vaccination against Covid-19 (recommended)
- Certificate III in Aged Care, Certificate IV in Ageing Support, Certificate III in Individual Support, Certificate IV in Leisure and Lifestyle, Diploma of Nursing, Allied Health Assistant qualifications, Health Care Worker qualifications or equivalent.
- Enrolled Nurse Only: Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as an Enrolled Nurse
- Demonstrated experience working with groups and conducting group based activities.




- Commitment to enhancing the quality of life of frail aged people, people with disabilities and their carers.
- Demonstrated ability to empower clients to participate in decision making and activity planning.
- Well developed interpersonal, communication and organisational skills
- Demonstrated ability to work within a team
- Demonstrated ability to use Microsoft Office software and other electronic communication and client records management software
- Current Food Handlers Certificate or willingness to obtain same
- Current Light Rigid Drivers Licence or willingness to obtain same
