Army and play a vital role in supporting veterans and their families who are experiencing or at risk of homelessness.

As a Case Manager, you'll provide person-centred, trauma-informed support that empowers individuals to secure safe housing, strengthen community connections and achieve long-term stability. Every day, you'll work alongside people to overcome barriers, build resilience and create positive, lasting change.

Working within our Homelessness Stream, you'll be part of a supportive team committed to delivering high-quality services that reflect The Salvation Army's mission, values and national Model of Care.

What you'll be doing In this rewarding role, you will:

Build trusted, respectful relationships that promote choice, independence and self-determination.

Deliver strengths-based, trauma-informed case management tailored to each person's unique goals and circumstances.

Complete holistic assessments, identifying needs, risks, strengths and barriers to housing stability.

Develop and implement individualised case plans in partnership with clients.

Support veterans and their families to access housing,



specialist services and community supports through effective advocacy and referrals.

Monitor progress, review outcomes and adapt support plans as circumstances change.

Maintain accurate case notes and records using The Salvation Army's information systems.

Identify and respond to risks while contributing to a safe and supportive service environment.

Work collaboratively with colleagues and external agencies to deliver coordinated, high-quality support.

What you'll bring

You'll be someone who genuinely cares about making a difference and enjoys working collaboratively to achieve positive outcomes.

You'll also have

A degree in Social Work, Community Services, Welfare or a related discipline, or equivalent experience and demonstrated sector knowledge.

Experience delivering case management within community services, with homelessness and/or veteran support experience highly regarded.

Strong assessment, communication and relationship-building skills.

The ability to manage complex situations using a person-centred, trauma-informed approach.

A current Queensland Working with Children Check (Blue Card), or the ability to obtain one.

A current Australian Driver Licence.
