My Place is seeking a passionate individual who is experienced in disability services to join our team as a Co-ordinator in the Lower Great Southern region.
In the role of Co-ordinator, you will work alongside people with disability, their families and allies to co-ordinate their NDIS supports in an individualised manner. Working closely with other Co-ordinators, you’ll help translate funding plans into practical supports, making sure budgets, bookings and claims are accurate, aligned and on track.
About My Place
My Place is an established not-for-profit provider of individualised and flexible supports for people with disabilities and their families. We empower around 400 individuals to live independently in their own homes or with their families, promoting their growth as valued and contributing members of the community. Importantly, we do not operate group homes, day centres, or other congregate care services.
As part of our team, you will be offered: a phone and laptop, flexibility for family commitments, 5 weeks of annual leave, and 13 weeks of long service leave after 7 years, along with other health and wellbeing perks.
About the Role
As a Co-ordinator, you will:
- provide co-ordination of supports, working alongside people with disability and their significant others to implement, monitor and review NDIS plans and achieve their goals.
- Work collaboratively with internal teams while providing leave coverage and supporting individuals and families to meet employment and engagement obligations.
- Maintain accurate records, documentation, budgets and funding claims, and liaise with relevant funding bodies and internal teams as required.
- Be based in the Lower Great Southern and travel in the region and to Perth as required
About You You’re organised, approachable and confident juggling multiple priorities. You care deeply about person-centred practice and are comfortable working both independently and as part of a team.
Selection Criteria
- Experience in disability services, NDIS co-ordination or a related field
- Experience in contemporary and innovative service design for individuals, and the implementation and ongoing management of individual support arrangements including funding and budgets
- Ability to work autonomously from a home office or in the community
- Solid systems skills and the ability to learn quickly
- Solid communication and relationship building skills
- Experience with budgeting, service documentation or relevant systems
Essential Criteria
- Demonstrated contemporary attitude and values toward people with disability including promotion of, and adherence to, My Place’s values and service charter (See: www.myplace.org.au).
- You must possess a current Drivers Licence
- The successful applicant will be required to obtain and hold a NDIS Worker Screening Clearence and complete the NDIS Worker Orientation Module
