PositionProfile: An opportunity exists for a Senior Social Workerto join our team in Halls Creek. In this advanced practice role,you will plan, coordinate, deliver and evaluate timely, culturallyappropriate social work services across Halls Creek and surroundingcommunities. Working within a primary health care framework, youwill support the promotion, maintenance and improvement of healthand wellbeing for individuals and families, providing clinicalsocial work services across both community and acute care settings.The position plays a key role in care coordination and dischargeplanning for clients with complex psychosocial needs, while alsodelivering secondary consultation, professional support and expertadvice to multidisciplinary teams and regional healthcare staff.This is a rewarding opportunity to make a meaningful impact in adiverse rural and remote community setting.

SuitabilityCriteria: Suitability recognises that a range of factorscontribute to merit, including skills, knowledge, abilities,behaviours, ethical alignment and relevant operationalrequirements. The suitability requirements below are derived fromthe Job Description Form and define what will be assessed todetermine your suitability for the role:
- Tertiaryqualification in Social Work and eligible for full membershipof the Australian Association of Social Workers.
- Demonstrated experienceand advanced level of skills and knowledge in the planning,implementation and evaluation of Social Work services inprimary health care, outpatient and acute settings, across thelifespan.
- Demonstrated knowledgeand understanding of primary health care principles andpractice.
- Demonstrated clinicalleadership and governance in Social Work services andpractice.
- Demonstrated leadershipworking in a cross-cultural setting, considering cultural andsocial determinants, particularly as they relate to Aboriginalhealth.
- Demonstrated personal,team and program/department managements skills withcommunication skills.
