In this advanced practice role you will plan, coordinate, deliver and evaluate timely social work services across Halls Creek and surrounding communities, within a primary health care framework.

You will support health and wellbeing, assist in discharge planning for complex cases, and provide expert advice to multidisciplinary teams in a diverse rural and remote setting.
