About the role
City Heart Medical is currently looking for a full-time Registered Nurse to work with our dedicated team of nurses, staff, and doctors providing the highest quality care in the role of Chronic Disease Management Nurse .
In this role, you will be expected to work both autonomously and collaboratively to deliver great patient care.
Duties will include all aspects of General Practice nursing with a strong focus on Care Plans and Reviews, Health Assessments, Patient Education and Follow Up etc.
What we're Looking For:
- You're a nurse with AHPRA registration and full working rights in Australia (no sponsorship available)
- You're experienced in GP Chronic Condition Management Plans and Health Assessments
- You’re passionate about patient care
- You hold a current CPR certificate - although assistance can be offered here if it has recently expired.
