We're looking for an experienced and passionate Program Manager – Home Care to lead Home Care and Respite teams, ensuring exceptional, person-centred services that make a genuine difference in the lives of older people.

This is a key leadership role within the Home Care division, working closely with senior leadership to drive service excellence, support multidisciplinary teams, and deliver cutting-edge, sustainable community care.

About the Role

As the Program Manager – Home Care, you will provide strategic and operational leadership to a small but dedicated team of aged care professionals. You will be responsible for ensuring high-quality, compliant and consumer-directed care while achieving operational and financial outcomes. You'll also play a key role in workforce leadership, service development and continuous improvement.

Key Responsibilities

- Lead and inspire multidisciplinary teams of professionals.
- Ensure high-quality, person-centred care that supports older people to remain independent.
- Oversee client assessment, care planning, service coordination and ongoing reviews.
- Drive customer satisfaction through responsive, respectful and goal-directed services.
- Manage budgets, resources and service performance to meet operational targets.
- Lead workforce planning, recruitment, coaching and performance development.
- Ensure compliance with the Aged Care Quality Standards, organisational policies and legislative requirements.
- Monitor quality, clinical governance, risk management and continuous improvement initiatives.




- Build strong relationships with clients, families, community partners and referral agencies.
- Support the growth of fee-for-service opportunities across the region.

About You

You are an experienced operational leader who is passionate about improving the lives of older people.

You will bring

- Tertiary qualifications in Management, Social Work or Nursing or a related discipline, or equivalent leadership experience within health or aged care.
- Demonstrated experience managing community aged care or home care services.
- Strong knowledge of assessment, care planning and service delivery for older people living in the community.
- Proven financial and budget management experience.
- Experience leading, developing and mentoring multidisciplinary teams.
- Sound understanding of risk management, clinical governance and quality systems.
- Excellent communication, relationship-building and stakeholder engagement skills.
- The ability to lead change, improve services and foster a positive team culture.
