About the role

As a Case Manager, you'll own a caseload of participants — coordinating their supports, tracking their goals, and making sure the right services are in place at the right time. This is a role for someone who is equal parts organised and people-focused: you'll be the steady point of contact who keeps everything moving, spots risks early, and advocates for the best outcomes.

You'll work day-to-day in our digital platform, so you'll need to be confident learning and using current systems rather than reaching for paper and spreadsheets.

What you'll be doing

- Managing a caseload of participants, coordinating their NDIS supports and monitoring progress against their goals

- Being the key point of contact for participants, families, guardians, and support workers

- Identifying and responding to changes in a participant's wellbeing, mental health, or circumstances, and escalating concerns appropriately

- Building and maintaining relationships with participants, referrers, allied health, and community stakeholders

- Keeping accurate, timely records in our digital platform and meeting NDIS documentation and compliance requirements

- Contributing to intake and onboarding of new participants, and helping the business grow through strong, trusted relationships





What you'll bring

Essential

- Experience in case management, support coordination, or a similar community services role

- A solid understanding of mental health, including recognising warning signs and responding with empathy and professionalism

- Confidence with technology — comfortable learning and working in digital systems, apps, and online tools

- Strong people and communication skills, with a background in sales, customer service, relationship management, or another people-facing field

- Excellent organisation and the ability to juggle competing priorities

- A genuine, values-driven approach to supporting people with disability

Desirable

- Experience in the NDIS or broader disability sector

- Qualifications in mental health, community services, social work, psychology, or a related field

- Understanding of psychosocial disability and recovery-oriented practice

Compliance requirements

To work with us, you'll need (or be willing to obtain):

- NDIS Worker Screening Check (WA)

- Working with Children Check (WA)

- Current First Aid & CPR certificate

- Valid WA driver's licence and reliable vehicle

- The right to work in Australia
