We're now seeking an experienced and compassionate Practice Nurse to join our team at the Althea Wellness Centre, where you'll play a vital role in improving the health outcomes of people experiencing complex health needs.

About the Role

As a Practice Nurse, you'll provide comprehensive primary healthcare to people whose health has been impacted by alcohol, tobacco and other drug use, mental illness and other complex health conditions.

Working within a collaborative multidisciplinary team, you'll deliver high-quality clinical care across both centre-based and outreach settings, supporting clients through health promotion, early intervention, chronic disease management and acute care.

This is a rewarding opportunity to combine your clinical expertise with a trauma-informed, harm minimisation approach, while working alongside passionate professionals committed to delivering integrated, person-centred care.

This position will be appointed at either Registered Nurse Level 2 or Registered Nurse Level 3, depending on the successful applicant's qualifications, skills and experience.

You will:

- Deliver comprehensive primary healthcare, including health assessments, screening, prevention and chronic disease management

- Provide clinical care including immunisations, venepuncture, pathology collection, wound care and vein care

- Support clients experiencing complex physical, mental health and AOD-related health concerns

- Work collaboratively with the Althea Wellness Centre multidisciplinary team to deliver coordinated, holistic care

- Participate in case reviews and contribute to integrated treatment planning

- Maintain accurate clinical documentation,



client records and reports

- Uphold professional, clinical and ethical standards in all aspects of service delivery

- Contribute to continuous quality improvement, accreditation and risk management processes

Local travel to different sites will be required as part of this role.

What We're Looking For

You're an experienced and compassionate Registered Nurse who is passionate about improving health outcomes for vulnerable communities. You value collaboration, thrive in a multidisciplinary environment and are committed to delivering respectful, evidence-based care.

You'll bring:

- Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA)

- Minimum five years' nursing experience, preferably within general practice, community health or working with vulnerable populations

- Demonstrated clinical competency in health assessments, immunisations, venepuncture, pathology collection, wound care and vein care

- Strong understanding of the health impacts associated with long-term alcohol and other drug use, including blood borne viruses and liver disease

- Knowledge of primary healthcare and the complex health needs experienced by vulnerable populations

- Understanding of the social determinants of health and their impact on wellbeing

- A commitment to trauma-informed, harm minimisation and person-centred practice

- Ability to work autonomously while contributing effectively within a multidisciplinary team

- Excellent communication, clinical documentation and computer skills, including experience with patient information management systems

- Commitment to reflective practice and continuous quality improvement

Requirements

All appointments are subject to:

✔ Current AHPRA Registration as a Registered Nurse
✔ Working with Vulnerable People Registration
✔ National Police Check
✔ Current Senior First Aid Certificate (or willingness to obtain)
✔ Australian Citizenship or suitable rights to work in Australia

A current Driver's Licence is desirable, and travel will be required as part of the role.
