Prospect Hill is seeking a hands-on Disability Team Leader to oversee two Supported Independent Living (SIL) homes in Townsville. This is a leadership role where you'll still work alongside participants and your team while providing guidance, mentoring and ensuring high-quality support is delivered every day.

About the Role

As the Team Leader, you'll be the consistent presence across two SIL homes, supporting participants while leading and mentoring your team to deliver safe, person-centred care.

You'll work closely with our COO, Compliance Manager and Head Office team, giving you the support you need to focus on quality outcomes for participants and staff.

Your responsibilities will include:

- Providing direct support to participants, including personal care, community access, medication assistance and daily living activities

- Leading,



mentoring and supporting Disability Support Workers during shifts

- Creating a positive, accountable and respectful team culture

- Monitoring participant support plans, behaviour support plans and risk strategies

- Coordinating appointments and maintaining communication with families and stakeholders

- Completing documentation, progress notes and incident reporting

- Conducting regular home checks and maintaining NDIS compliance standards

- Identifying issues early and working proactively to resolve them

About You

We're looking for someone who enjoys supporting people just as much as they enjoy supporting a team.

You'll bring:

- Experience working within Supported Independent Living or disability services

- Leadership experience, or the confidence and capability to step into a leadership role

- Excellent communication and organisational skills

- A calm, positive and solution-focused approach

- A commitment to providing high-quality, person-centred support

Essential Requirements

- Current NDIS Worker Screening Check

- Current First Aid & CPR

- Current Driver Licence and reliable vehicle with comprehensive insurance

- Unrestricted Australian working rights

Desirable

- Certificate III or IV in Individual Support (Disability) or similar

- Understanding of NDIS Practice Standards and SIL operations
