We’re seeking a Registered or Enrolled Nurse to join our Modbury Medical Centre , where you’ll support GPs and patients through chronic condition management and preventative healthcare . You’ll be part of a great, supportive team that values collaboration, culture, and patient outcomes. Part-time and full-time opportunities available.
Responsibilities
- Prepare Care Plans and Team Care Arrangements
- Support patients in managing chronic conditions and improving health behaviours
- Conduct health assessments and promote preventative care
- Collaborate with GPs and multidisciplinary teams
- Manage administrative tasks related to CCM appointments
Qualifications
- AHPRA registered Enrolled or Registered Nurse
- Prior experience in Chronic Condition Management preferred
- Strong communication and organisational skills
- Friendly, proactive, and patient-focused approach
