We are now seeking a compassionate and dedicated Registered Nurse to join our Support at Home Team, providing high-quality clinical care and support to older people living independently in the community.
This rewarding role offers the prospect to build meaningful relationships with clients while promoting their health, wellbeing, independence, and quality of life in their own homes.
About the Role
As a Registered Nurse within the Support at Home Team, you will deliver person-centred nursing care, conduct clinical assessments, develop care plans, and work collaboratively with clients, families, carers, and multidisciplinary teams to achieve positive outcomes.
You will be responsible for managing a varied caseload across the community, ensuring care is delivered safely, effectively, and in accordance with professional standards and organisational policies.
Key Responsibilities
- Conduct comprehensive nursing and health assessments.
- Develop, implement, and review individualised care plans.
- Provide clinical care including wound management, medication management,
continence support, and chronic disease management.
- Monitor clients' health and respond appropriately to changes in condition.
- Support clients to maintain independence and achieve their health goals.
- Provide education and support to clients, families, and carers.
- Liaise with General Practitioners, Allied Health professionals, and other healthcare providers.
- Maintain accurate clinical documentation and reporting.
- Participate in quality improvement initiatives and clinical governance activities.
- Ensure compliance with relevant aged care standards, legislation, and professional nursing requirements.
About You
The successful candidate will demonstrate strong clinical assessment skills, excellent communication and interpersonal abilities, and a commitment to delivering high-quality, person-centred care. You will be comfortable working autonomously within the community while also contributing as a valued member of a multidisciplinary team.
Maintaining accurate clinical documentation, monitoring client health outcomes, and ensuring compliance with relevant aged care standards, legislation, and professional nursing requirements will be essential components of the role.
Essential Skills & Experience
- Current registration with the Nursing and Midwifery Board of Australia (AHPRA)
as a Registered Nurse.
- Demonstrated experience in community nursing, aged care, primary health care, or a related clinical setting.
- Excellent communication and relationship-building skills.
- Strong clinical assessment and care planning skills.
- Excellent communication and interpersonal abilities.
- A commitment to delivering person-centred care.
- Ability to work autonomously while collaborating effectively within a multidisciplinary team.
- Sound computer and documentation skills.
- Current Driver's Licence and willingness to travel within the community.
- Current National Police Check and relevant clearances (or willingness to obtain).
Desirable
- Experience in community aged care.
- Knowledge of the Aged Care Quality Standards.
