We encourage people of Aboriginal and Torres Strait Islander background and people with disability to apply.About the RoleAs a Care Partner, you will coordinate and deliver high-quality aged care services across Rockhampton and surrounding areas. You will manage client referrals, complete care assessments, develop and review care plans, and conduct home visits to ensure clients receive personalised support.You will build strong relationships with clients, families, and stakeholders while helping to improve day-to-day processes and ensure a positive client experience.This is a permanent full time role based in Rockhampton, with regular travel throughout the region for home visits. It's a great opportunity for a motivated professional looking to grow their career in aged care and make a meaningful difference.Key ResponsibilitiesConduct client assessments and develop care plans with clients, representatives, and stakeholdersUpdate care plans regularly and conduct home visitsMonitor clinical risks and client needs, ensuring appropriate referral pathways are in placeWork collaboratively with internal teams,



including Rostering and Practice Leads, to meet client needsDevelop strategies and/or broker services to address the identified needs of Aboriginal and Torres Strait Islander clients in consultation with families and communitiesRepresent Life Without Barriers at external forums and meetings, promoting the organisation within the community
Skills & Experience Demonstrated Aged Care case management experienceUnderstanding of Aged Care funding and service delivery, specifically Support at HomeConfidence in utilising a variety of technology including mobile devices and computersAbility to effectively build relationships with diverse stakeholdersStrong communication skillsAbility to multi-task and work autonomously in a fast-paced environmentA current unrestricted Australian Drivers Licence, reliable vehicle and a willingness to travel
