Job responsibilities:

- Support safe transitions of care, including handoffs and care plan updates.
- Perform focused reassessments and escalates concerns through appropriate clinical channels.
- Document assessments, interventions, and patient progress in the medical record.
- Work 36 hours per week with 13-hour shifts.
- Assess patient status and respond to changes using established clinical protocols.
