Gems Care is seeking experinced Mentor Support Workers (Disability Support Workers) to join our growing team on a permanent part time basis, supporting participants from Surfside to Moruya region. This position offer a minimum of 50 hours a per a fortnight that will require flexibility to work a rotating roster which includes shift work, overnight sleepover shifts, weekend work and working on public holidays.

We are looking for compassionate, reliable, and experienced support workers who are passionate about empowering people with disability to live independently and achieve their goals within their local community.

Key Responsibilities

· Deliver person-centred supports tailored to each participant's individual goals and needs.

· Assist participants to develop daily living, social, employment, and recreational skills.

· Promote community participation, inclusion, independence, and informed decision-making.

· Provide personal care, medication assistance, meal preparation, domestic assistance, transport, and community access as required.

· Complete accurate progress notes, documentation, and incident reports.

· Build positive and professional relationships with participants, families, advocates, and allied health professionals.





· Mentor and encourage participants to build confidence and maximise their potential.

· Maintain a safe, respectful, and supportive environment at all times.

Qualifications

· Minimum Certificate III in Individual Support, Disability, or an equivalent qualification.

Experience

· Demonstrated experience supporting people with disability.

· Experience in manual handling (desirable).

· Previous experience working within the NDIS sector is highly regarded.

Essential Requirements

· Current NDIS Worker Screening Check.

· Current Working with Children Check (where applicable).

· Current First Aid and CPR Certificate.

· Current Australian Driver Licence and access to a reliable, comprehensively insured vehicle.

· Completion of the NDIS Worker Orientation Module.

· Current right to work in Australia.

· Willingness and ability to travel to support participants across the Surfside to Moruya region
