The Role

As a Clinical Care Coordinator, you will be an experienced Registered Nurse providing

essential clinical support to older adults living in their homes, ensuring safe, effective,

and holistic care that promotes well being, independence, and quality of life. Working

closely with the Clinical Services Manager, you will ensure clients receive safe,

person‑centred care aligned with their goals, health needs and funding programs. You

will also provide guidance, supervision, and support to a team of Enrolled Nurses.

Additionally, you will;

- Provide clinical oversight, supervision, and mentoring for Enrolled Nurses, ensuring

safe practice and adherence to organisational and professional standards.

- Conduct comprehensive assessments, develop and review care plans, and monitor

client clinical needs in alignment with their health goals and personal preferences

- Engage regularly with Care Coordinators to ensure clinical interventions are aligned

with client needs and funding requirements

- Deliver direct nursing care including wound care, chronic disease management,

medication management, and health education

- Provide timely responses to changes in client health status, including initiating

appropriate interventions or referrals

- Maintain strong communication with clients, families, caregivers, allied health

professionals, GPs, Care Coordinators and external providers

- Assist in delivering training to relevant team members on areas such as wound care,

clinical assessment, medication administration, and infection prevention.

- Review and evaluate treatment plans regularly to ensure that goals and clinical

needs are met

- Maintain clinical records,



client documentation and provide information and report

on services delivered

- Participate in ongoing professional development and contribute to service

improvement initiatives.

What We're Looking For

- Relevant Degree qualification in Nursing

- Current AHPRA registration

- A demonstrated understanding of community aged care nursing is desirable

- Previous experience in the Aged Care sector is desirable but not essential

- Able to build trusting, working relationships throughout the organisation to support

a collaborative, multidisciplinary approach

- Exceptional written and verbal communication skills with a strong focus on client

outcomes
