Are you passionate about community development, social inclusion and creating meaningful opportunities for people to stay connected and engaged? Join Western District Health Service as a Community Development Worker – Social SupportGroups, where you’ll play a key role in enhancing the wellbeing, independence and community participation of older people and eligible community members across Hamilton, Penshurst and Coleraine.

This is a role for someone who values collaboration, innovation and community engagement and who wants to make a genuine difference by developing opportunities that foster social connections, strengthen inclusion and support people to remain active participants in community life.

What You'll Be Doing

As part of the Social Support Group team, you'll help create meaningful opportunities that support independence, wellbeing and social connection for older people and eligible community members across Hamilton, Penshurst and Coleraine.

Community Engagement & Relationship Building —
Develop and maintain positive relationships with members, families, volunteers, community organisations, schools, contractors and internal staff to strengthen community participation and social inclusion.

Program Promotion & Representation —
Represent and promote the Social Support Group program through community meetings, events, presentations, networking activities and other engagement opportunities within the wider community.

Project & Event Coordination —
Plan, organise and coordinate community development initiatives, projects, activities and events, managing multiple priorities and competing deadlines to achieve positive outcomes for members.

Member Consultation & Continuous Improvement —
Gather, analyse and document member feedback, program information and outcomes to support program evaluation, environmental improvements and ongoing service development.

Administration & Communication —
Undertake a range of administrative responsibilities including data entry, record keeping, report writing and electronic communication to support effective program delivery.

Partnership Development —
Work collaboratively with community organisations, stakeholders and intergenerational partners to identify and develop opportunities that enhance wellbeing, foster community connections and encourage active participation.

Teamwork & Collaboration —
Work both independently and as part of a multidisciplinary team, contributing to a positive,



supportive and collaborative workplace culture.

Problem Solving & Adaptability —
Respond to changing priorities, identify solutions to emerging issues and exercise sound judgement to meet member, program and community needs.

Community-Based Service Delivery —
Travel independently between Hamilton, Penshurst, Coleraine and other community locations, participating in activities that may require periods of sitting, standing, walking, bending, reaching and driving.

What You Bring

- Demonstrated experience in community development, community engagement or a related field
- Strong ability to build and maintain effective relationships with community organisations, schools, volunteers, members and other stakeholders
- Commitment to person-centred practice, ensuring members are actively involved in planning, decision-making and program development
- Experience developing, coordinating and delivering community initiatives, projects and events that promote social inclusion and participation
- Excellent written, verbal and interpersonal communication skills, with the ability to promote programs and represent WDHS within the community
- Proven organisational skills, with the ability to manage competing priorities, coordinate multiple activities and achieve outcomes within agreed timeframes
- Ability to gather, analyse and apply feedback and outcome data to support program evaluation, innovation and continuous improvement
- Ability to work collaboratively within multidisciplinary teams, exercise sound judgement and adapt to changing community and program needs.
