What you will do:
Provide clinical pharmacy services within NHS hospital wards




Conduct thorough drug history taking and medicines reconciliation
Undertake clinical screening and TTA screening
Utilize EPMA systems effectively and contribute to safer prescribing practices
Engage with multidisciplinary teams to offer expert pharmaceutical advice
Ensure the optimal use of medicines and patient safety
Attend and contribute to clinical meetings and case discussions

Requirements:
NHS experience
Hospital ward-based clinical pharmacy experience
Competency in drug history taking and medicines reconciliation
Clinical and TTA screening expertise
Essential EPMA system experience
Preferred experience with the Cerner system, as it’s used by the Trust
Previous experience within a London Acute NHS Trust
Experience with the Cerner EPMA system adds a distinct advantage
