As a Reablement OT, you will play a crucial role in assisting individuals after hospital discharge to regain their independence, working 37 hours per week, based at Bourne Hill, Salisbury. What you will do:
Conduct thorough assessments for individuals following hospital discharge.
Develop and implement tailored reablement plans to support independence.
Provide expertise on adaptive equipment and safe practices.
Collaborate with the wider team to support decision‑making processes.
Manage your caseload efficiently by prioritising tasks and maintaining accurate records.
Spend around 50% of your time in the office conducting administration work, with the remaining time divided between home working and community visits.
Fulfil the needs of your role with the requirement of a full UK driving licence.
Requirements:
Ability to travel independently for community visits is required.
Full UK driving licence is essential.
