UnitingCare Queensland is seeking a Supported Living Facilitator to provide in-home personal care and daily living assistance to clients living in the community. You will support independence and improve quality of life with adaptable schedule, typically Monday to Friday, 9am-5pm.
You will work as part of a compassionate team, delivering care under individual care plans and assisting with transportation and social activities. A driver’s licence and access to a vehicle are required.
