About the Role
We are seeking caring and reliable Home Care Worker to support our clients living in the Brisbane South region. These clients are typically transitioning from hospital back to home and require support.
Key Responsibilities
Personal care — showering, grooming, toileting
Social support and companionship
Transport support — appointments, shopping, community activities
Domestic assistance — cleaning, laundry, meal prep
Supporting Transitional Care clients with structured routines and recovery‑focused goals
What You Need
Certificate III in Individual Support (Ageing/Disability) or equivalent
A caring, reliable, and skilled approach
Solid communication skills with clients, families, and the care team
Patience, empathy,
and a genuine commitment to improving seniors’ wellbeing
Current National Police Check (or willingness to obtain)
Confident working independently
Current First Aid & CPR (or willingness to obtain)
Open Australian Driver Licence
Reliable, registered vehicle with comprehensive insurance
Willingness to travel between clients
Permanent Australian Residency or valid work rights
Early‑morning availability (essential for Transitional Care clients)
