We are seeking an experienced Community Services Coordinator to manage a caseload of Home Care Package and NDIS clients across Perth, Rockingham, Mandurah, and Bunbury. This role focuses on coordinating person‑centred services that promote clients' independence, wellbeing, and achievement of individual goals while living in the community.

You will be part of our dedicated Community Services team, delivering exceptional support to older adults and people living with disability. This is a non‑clinical role that involves working closely with clients, families, support workers, allied health specialists, and other stakeholders to ensure high‑quality, consumer‑focused services.

Location:

You will be based at our Perth office and will provide services across Rockingham, Mandurah, and Bunbury. Regular travel to client appointments is required, making this role well suited to candidates living in Perth's southern suburbs.

Key Responsibilities
Developing and maintaining robust,

Building and maintaining referral networks with external stakeholders.
Conducting client assessments, reviews, and care planning using a consumer‑directed approach.
Coordinating quality, secure, and person‑centred services within individual client budgets.
Managing a caseload of Home Care Package and NDIS clients.
Providing guidance and support to Support Workers as required.
Liaising with the Community Services and Nursing teams to ensure appropriate service delivery.
Participating in quality improvement initiatives and continuous service development.
Assisting with administrative duties as directed by the Line Manager.
