Position Summary

The Service Coordinator is responsible for coordinating the delivery of NDIS services, ensuring participants receive high-quality, person-centred supports. The role oversees workforce scheduling, participant onboarding, service delivery, compliance, and stakeholder communication while maintaining a high standard of customer service.

Key Responsibilities
Coordinate and manage the delivery of NDIS supports for participants.
Develop and maintain staff rosters to ensure adequate service coverage.
Match participants with suitable support workers based on their needs and preferences.
Conduct participant intake, assessments, and service commencement.
Maintain regular communication with participants, families, carers, and support workers.
Monitor service quality and participant outcomes.
Resolve rostering issues, service changes, and emergency staffing requirements.
Ensure compliance with NDIS Practice Standards, organisational policies, and safeguarding requirements.
Maintain accurate participant records, case notes, incident reports,



and service agreements.
Support recruitment, onboarding, and ongoing supervision of support workers.
Investigate and manage complaints, incidents, and participant feedback.
Prepare reports and contribute to continuous quality improvement initiatives.
Essential Skills and Experience
Experience in disability, community services, aged care, or healthcare coordination.
Solid rostering and workforce management experience.
Excellent communication, interpersonal, and conflict-resolution skills.
Ability to prioritise tasks and manage multiple participants and staff simultaneously.
Knowledge of the NDIS, disability sector, and person-centred practice.
High level of computer literacy, including Microsoft Office and client management systems.
Robust organisational and problem-solving skills.

Pay: $85,000.00 – $95,000.00 per year
