This is a combination role for someone who loves building real relationships with participants and their families, while also being the kind of person who keeps compliance, records, and admin ticking along without missing a beat. You'll manage a caseload of aged care participants; developing and reviewing care plans, coordinating services, and being their trusted point of contact while also handling the administrative backbone that supports excellent care: referrals, invoicing, compliance tracking, and reporting.
What you'll be doing:
Developing, reviewing and coordinating individualised care plans in partnership with participants and their families
Building solid relationships with GPs, allied health professionals and other service providers
Monitoring changing participant needs and escalating clinical risks where required
Managing referrals, invoicing, budgets and compliance documentation
Keeping participant files accurate, audited and up to date
What we're looking for:
Experience in case management, care coordination, or aged/disability care administration
Comfortable juggling relationship-based work with detail-focused admin
Robust communication and organisational skills
