Provide person-centred support to NDIS participants in line with individual care plans and personal goals.
Assist with personal care, including showering, dressing, grooming, toileting, and continence support.
Support participants with mobility, transfers, and the safe use of mobility equipment.
Prepare nutritious meals and assist with feeding where required.
Provide medication prompts in accordance with care plans and organisational policies.
Complete household tasks, including cleaning, laundry, and maintaining a secure living workplace.
Transport participants to medical appointments, shopping, and community activities.
Encourage independence, choice, dignity, and active participation in daily living.
Support participants to engage in social, recreational, and community activities to promote inclusion and wellbeing.
Monitor participants’ health and wellbeing, reporting any changes or concerns promptly.
Maintain accurate progress notes and documentation.
Communicate effectively with participants, families, and allied health specialists.
Follow NDIS Practice Standards, organisational policies, and Work Health and Safety (WHS) requirements.

Pay: $33.49 – $52.39 per hour
