Assured Independent Living Services (AILS) is a registered NDIS provider servicing participants across Sydney.
We are looking for a passionate and experienced support coordinator to join our growing organisation.
Key Responsibilities
Coordinate and implement participants' NDIS plans.
Build participants' capacity to understand and manage their supports.
Develop and maintain robust relationships with participants, families, carers, and stakeholders.
Connect participants with suitable service providers and community resources.
Monitor service delivery and ensure supports align with participant goals.
Advocate for participants and support informed decision-making.
Maintain accurate case notes and documentation in accordance with NDIS Practice Standards.
Prepare reports and liaise with the NDIA and other stakeholders as required.
Manage a caseload while ensuring high-quality, participant-focused outcomes.
About You
To be successful in this role, you will have:
Demonstrated experience as an NDIS Support Coordinator.
Robust knowledge of the NDIS, relevant legislation, and the disability sector.
Excellent communication, negotiation, and interpersonal skills.
Strong organizational and time management skills.
Ability to work independently and effectively manage your own workload.
A genuine commitment to person-centered practice and participant empowerment.
Solid problem-solving and advocacy skills.
Experience using client management systems and maintaining accurate records.
