Lumia Care is seeking an experienced and compassionate Clinical Care Partner to join our in‑home aged care team in Dulwich, SA. This is a key role supporting older Australians to remain independent, safe, and respected in their own homes.

About the Role

As a Clinical Care Partner, you will work autonomously in the community while collaborating with a dedicated and supportive team. You will coordinate personalised in‑home care for participants funded under the Australian Government's Support at Home programme. This role combines clinical expertise, care coordination, and stakeholder engagement to ensure high‑quality, compliant, and person‑centred service delivery.

Key Responsibilities
Conduct home visits to assess participant needs, including safety and mobility assessments, and develop individualised care plans
Build strong relationships with participants, families, and health specialists to deliver tailored, outcome‑focused care
Coordinate services, manage handovers, and ensure care plans, budgets, and supports are accurately documented and delivered
Maintain compliance with the Aged Care Act 2024, relevant standards, and internal policies
Respond to participant feedback and complaints,



ensuring timely resolution and positive outcomes
Support continuous improvement initiatives, including quality assurance, policy development, and accreditation activities
Attend the office weekly to collaborate with the team and contribute to operational alignment

What You Bring
Registered or Enrolled Nurse with minimum 3 years' experience in aged care, community, or health services
Current AHPRA registration and NSW Driver's Licence
Strong understanding of Commonwealth in‑home aged care programmes, legislation, and standards
Well‑developed clinical judgement and stakeholder management skills
Confidence using multiple systems, portals, and Microsoft Office
A genuine commitment to delivering high‑quality, participant‑centred care
