In the Transition Care Program (TCP) at UnitingSA- We are about
restoration and recovery- right when an older person leaves the
hospital, helping them achieve their personal goals and regain
their independence so they can live confidently at home.
We're on
the lookout for a collaborative, person-centred, and organised Care
Partner to join our tight-knit, fast-moving TCP team.
What You'll Do
As a Care Partner within the Transition Care Program (TCP), you
are responsible for managing client recovery plans, which span a
period of 6 to 12 weeks.
You will:
Work Collaboratively - Co-design personalised, goal-based
restorative programs directly with clients, their families, and
clinical teams.
Navigate the System - Connect clients to a wide web of
clinical services, including Allied Health, nursing and broader
community aged care services.
Work within timeframes - You will manage short-term
schedules, coordinate efficient discharges, and work within
specific funding parameters and program guidelines.
Set Real Expectations - Keep communication transparent,
helping clients and families understand exactly what the
program can achieve and problem solving along the way.
What You'll Bring
Program Framework and Aged Care Knowledge:An understanding of the Transition Care Program (TCP)
framework, or the ability to learn it rapidly, combined with a
comprehensive understanding of the diverse health and wellness
needs of older people.
Community Care Experience:A solid
understanding of the community aged care system.
Familiarity
with Support at Home and Commonwealth Home Support Programme
(CHSP)



guidelines is essential.
Communication Skills:Excellent
interpersonal and communication skills, with the ability to
engage empathetically, clearly, and respectfully with a diverse
range of clients and stakeholders, including clinicians,
hospital discharge planners clients and their family
members.
Person-Centered Approach:A strong
commitment to ensuring older individuals receive care that
prioritizes their choice, dignity, and tailored support.
Time Management:Strong organizational
skills and the ability to work effectively within strict,
time-critical parameters and program deadlines.
Qualification in Aged or Community Support or equivalent
qualification.
Familiarity with electronic client management systems.
Great in computing skills and ability to learn new
systems.
Desirable Criteria
Prior experience working specifically within a Transition
Care Program (TCP).
A clinical qualification in Nursing, Allied Health, or a
related field is well regarded but not essential.
