We are seeking an experienced and passionate Nurse to join our Community Treatment Team at Cockburn Mental Health.
As aClinical Nursewithin the Community Treatment Team, you will deliver advanced nursing care and care coordination for patients requiring treatment outside of the hospital environment.
Working as part of a multidisciplinary team, you will assess, plan, implement, and evaluate person-centred care while supporting patients and their families through their healthcare journey.
The successful applicant shall be part of a multidisciplinary team provides clinical and professional expertise to ensure comprehensive evidence-based nursing recovery focused care is delivered to patients.
Facilitates and promotes patient safety and quality of care in the community setting.
TheClinical Nursepractices within their scope of practice considerate of the Nursing and Midwifery Board's Nursing Practice Decision Flowchart.

South Metropolitan Health Service (SMHS) Mental Health
delivers comprehensive public mental health care through a network of community mental health services, hospitals and community health centres.
Across Fiona Stanley Fremantle Hospital Mental Health (FSFH Mental Health) and Peel and Rockingham Kwinana Mental Health (PaRK Mental Health), specialist services are provided across the South Metropolitan region, supporting consumers with accessible, collaborative and high-quality mental health care.
SMHS Mental Health will soon open a new state-of-the‐art facility at Fremantle Hospital designed to offer expanded and modern mental health services for our community.
The three-storey space will provide an additional 40 mental health beds and a 24-hour Mental Health Crisis Centre, ensuring compassionate and innovative care for those who need it most.
Cockburn Health
is a stand‐alone 75 bed inpatient facility.
The hospital is non‐authorised hospital, and for voluntary female patients only.
FSH Cockburn Health provides inpatient services for:
Women's Mental Health.
Eating Disorders.
Alcohol and Other Drug (AOD) Behavioural and Withdrawal Management.
