Due to continued growth, they are seeking an experiencedSupport Coordinatorto join their friendly team on a part-time basis, supporting participants across South Brisbane and surrounding areas.
About the Role and your Responsibilities:
As a Support Coordinator, you will work closely with NDIS participants, their families, and support networks to help them understand and implement their NDIS plans.
You will connect participants with appropriate services, build their capacity to manage supports independently,



and advocate for positive outcomes while maintaining a person-centred approach.
Some of your responsibilities will include:
Support participants to understand and implement their NDIS plans
Coordinate and connect participants with appropriate service providers and community supports
Develop strong relationships with participants, families, carers and stakeholders
Monitor participant progress and ensure supports continue to meet their goals and changing needs
Maintain accurate case notes, reports and documentation in line with NDIS requirements
Advocate for participants to ensure they receive the appropriate supports and services
Liaise with allied health professionals, service providers and other stakeholders to achieve positive outcomes
Manage a varied caseload while maintaining a high level of customer service and professionalism
Skills and Qualifications:
Previous experience as a Support Coordinator within the NDIS sector is essential
Strong understanding of the NDIS and the broader disability sector
Excellent knowledge of NDIS legislation, funding and participant plans
Outstanding communication and relationship-building skills
Highly organised with excellent time management and the ability to manage competing priorities
Strong problem-solving skills and the ability to think creatively to achieve positive participant outcomes
Experience working collaboratively with a wide range of stakeholders
Ability to work autonomously while contributing positively to a supportive team environment
Current driver's licence and reliable vehicle
NDIS Worker Screening Check and Working with Children Check (or willingness to obtain)
