Their experienced and supportive team works collaboratively to deliver tailored support that empowers participants to build independence, increase capacity, and achieve meaningful outcomes.Due to continued growth, they are seeking an experienced Support Coordinator to join their friendly team on a part-time basis, supporting participants across South Brisbane and surrounding areas.About the Role and your Responsibilities:As a Support Coordinator, you will work closely with NDIS participants, their families, and support networks to help them understand and implement their NDIS plans.
You will connect participants with appropriate services, build their capacity to manage supports independently,



and advocate for positive outcomes while maintaining a person-centred approach.
Some of your responsibilities will include:Support participants to understand and implement their NDIS plansCoordinate and connect participants with appropriate service providers and community supportsDevelop strong relationships with participants, families, carers and stakeholdersMonitor participant progress and ensure supports continue to meet their goals and changing needsMaintain accurate case notes, reports and documentation in line with NDIS requirementsAdvocate for participants to ensure they receive the appropriate supports and servicesLiaise with allied health professionals, service providers and other stakeholders to achieve positive outcomesManage a varied caseload while maintaining a high level of customer service and professionalismSkills and Qualifications:Previous experience as a Support Coordinator within the NDIS sector is essentialStrong understanding of the NDIS and the broader disability sectorExcellent knowledge of NDIS legislation, funding and participant plansOutstanding communication and relationship-building skillsHighly organised with excellent time management and the ability to manage competing prioritiesStrong problem-solving skills and the ability to think creatively to achieve positive participant outcomesExperience working collaboratively with a wide range of stakeholdersAbility to work autonomously while contributing positively to a supportive team environmentCurrent driver's licence and reliable vehicleNDIS Worker Screening Check and Working with Children Check (or willingness to obtain)Perks:Attractive salary of $75,000 to $85,000 + Super depending on experienceFlexible part-time hours to support work-life balanceSupportive leadership team committed to your successOngoing Professional Development and training opportunitiesOpportunity to make a genuine difference in the lives of participants every dayBe part of a growing organisation with a strong focus on collaboration, quality and participant outcomes
