About the role
As Service Mapping & Intake Officer, you'll sit at the heart of HPC's Spoke service, working alongside our centralised Telephony Intake and Assessment Hub to triage referrals and connect clients across the Hunter, New England and Central Coast region with the most appropriate care - quickly, safely and seamlessly.

- Build and maintain a comprehensive, up-to-date directory of services across the region
- Triage referrals from the Hub, GPs and other referrers, matching each client to the right service for their level of care and needs
- Conduct phone-based risk assessments, including suicide risk and AOD assessments, and escalate complex or high-risk referrals without delay
- Build strong, lasting relationships with service providers to keep referral pathways efficient and client-focused
- Champion the Spoke service across the region through proactive engagement with GPs and Local Health Districts

Skills & experience

- Tertiary qualification in Social Work, Psychology, Occupational Therapy or Mental Health Nursing,



and/or significant equivalent experience in mental health intake, triage and referral coordination
- Solid understanding of the mental health, AOD, psychosocial, homelessness and family/domestic violence service systems
- Confidence conducting risk assessments, including suicide risk assessment, within required timeframes
- Excellent communication and negotiation skills, including experience liaising with GPs and Allied Health providers
- Sound judgement and the ability to make timely, effective decisions in a fast-paced environment
