Mental Health Nurse Baldivis Family Medical Centre Rockingham, Western Australia, Australia
We are seeking a compassionate and experienced Mental Health Nurse to join our busy and patient-focused medical practice. This is an exciting opportunity to work as part of a multidisciplinary healthcare team, supporting patients with their mental health and wellbeing needs in a primary care setting.
Key Responsibilities
- Conduct mental health assessments and ongoing patient reviews.
- Provide support, education, and advocacy for patients and their families.
- Assist with the development and implementation of Mental Health Treatment Plans.
- Coordinate care with General Practitioners, psychologists, psychiatrists, and other allied health professionals.
- Support patients with chronic mental health conditions through care planning and follow-up.
- Assist in the planning, promotion, and facilitation of mental health workshops, group programs, and educational sessions for patients and the community.
- Provide mental health education and resources to patients, carers, and staff.
- Maintain accurate and confidential clinical records.
- Participate in quality improvement activities and multidisciplinary team meetings.
Essential Criteria
- Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (AHPRA).
- Qualifications and/or experience in mental health nursing.
- Excellent communication, presentation, and interpersonal skills.
- Ability to facilitate group discussions and educational sessions.
- Ability to work independently and as part of a multidisciplinary team.
- Strong organisational and time management skills.
- Understanding of confidentiality, privacy, and professional boundaries.
Desirable Criteria
- Credentialed Mental Health Nurse status or eligibility.
- Experience working in general practice, community mental health, or primary care settings.
- Experience delivering mental health education, workshops, or group programs.
- Knowledge of Medicare-funded mental health services and care coordination programs.
