The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.

The Regional and Specialist team includes Integrated Specialist Services, Mental Health and Wellbeing Services, Primary Healthcare Services, Birthing and Early Childhood Services (South), and Child and Youth Services. Our purpose is to connect, guide, and empower teams and communities through culturally protected, transparent, and responsive ways.

About the Role?

As Care Coordinator CCSS, your role is to implement the Care Coordination and Supplementary Services elements of the Integrated Team Care (ITC) program, with mainstream General Practices. The Regional Care Coordinator will work collaboratively with patients and designated general practices to facilitate a person with a chronic condition to manage their health in a way that will result in an optimal health outcome. The Regional Care Coordinator will also provide direct/relief Care Coordinator services as required. Working collaboratively with patients and ACCHS’s to facilitate a person with a chronic condition to manage their health in a way that will result in an optimal health outcome and as required, facilitate Aged Care eligibility and needs assessment processes and contribute to the development of client care plans.

Your day to day duties may include:

- Liaise and consult with target Aboriginal and Torres Strait Islander communities and health services to collaboratively assess health needs and priorities and subsequently facilitate appropriate Chronic Disease medical specialist service delivery arrangements that address identified issues, within the designated area.




- Develop networks in Regional Indigenous Communities to build community support for promotion and participation in Chronic Disease programs and treatment.
- Improve case management of referral and treatment by culturally appropriate specialist Medical Services and Allied Health Services to eligible Chronic Disease patients.
- Organise CPAP Clinic dates, recommended review dates for clients on a CPAP machine and organise set up appointments for new CPAP clients.
- Assist patients to; adhere to treatment regimes, develop chronic condition self-management skills e.g. BSL daily, recording BSL, checking feet/legs for ulcer Connect and liaise with community-based services, coordinate home visits as required.

About You?

- Current Criminal History Check.
- Current C Class Drivers Licence (QLD).
- Current AHPRA Registration
- Current Disability Worker Screening Check (Qld Yellow Card).
- Registered or Endorsed Enrolled Nurse.
- Qualification in Nursing.
- Ability to travel across SEQ

Desirable, but not mandatory:

- Demonstrated experience working with Aboriginal and Torres Strait Islander people and /or communities
