ISLHD is seeking a Social Worker for ITACS at Shoalhaven Memorial Hospital to provide social work and case management from admission to discharge, with early psycho-social assessments and care planning for elderly clients in the home.

You will liaise with families, carers, community agencies and the client’s GP, including counselling and carer support as part of an inter-disciplinary team delivering low intensity home-based therapy.
