## NDIS Support CoordinatorApplylocations: Windsor, Queenslandtime type: Full timeposted on: Posted Todayjob requisition id: JR **Join an Organisation dedicated to Closing the Gap in Indigenous Health****About Us?**The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.The Disability Services team lead the IUIH NDIS pilot project that will ensure Aboriginal and Torres Strait Islander people with disability aged 0-64 years in service areas of south-east Queensland are individually assisted and are supported by the Disability Services Support Coordination team to utilise their NDIS plan funds for services that the participant wants and needs. IUIH is committed to breaking down barriers and support Aboriginal and Torres Strait Islander people in an endeavour to address the unacceptably low rate of Aboriginal and Torres Strait Islander participant numbers accessing their funding under the NDIS.**About the Role?***Opportunity available for qualified or junior applicants – Full time or part time (minimum 4 days per week).*As a NDIS Support Coordinator, you will be responsible for assisting NDIS participants in interpreting their NDIS plan, assisting them to implement the supports in their plan, managing their budget and developing their capacity over time to coordinate and manage their own supports. You will play an integral role in supporting participants, which includes:* Ensuring participants are linked in with the IUIH System of Care,



to ensure the holistic needs of the participant are met.* Working with participants and their families to improve their confidence, knowledge, and skills to coordinate and manage their own supports (where appropriate).* Assisting participants with high or multiple needs to identify strategies to meet their NDIS and overall health and wellbeing goals.* Developing an individual support plan that identifies clear goal outcomes, in partnership with participants.* Building and maintaining your internal and external networks to assist in coordinating the culturally safe and appropriate support for your clients.* Identify and connect participants with community and mainstream services who will support the participant to improve on their overall health, wellbeing and quality of life.* Identify and connect participants with allied health services that will provide therapeutic supports, aligning with the Participants NDIS and, self-determined goals.* Collaborating with multiple stakeholders and the participants care team to ensure they are receiving high quality,
