Sirv is looking for an exceptional Team Leader to join our team in HORSHAM , Victoria, overseeing our Supported Independent Living (SIL) home and ensuring high-quality care for our participants.

About The Role

- Oversee the daily operations of our Supported Independent Living homes.
- Develop and implement support plans and risk assessments tailored to participants' individual needs and goals.
- Ensure restrictive practices reporting is completed accurately and submitted to relevant authorities (e.g., NDIS, DHHS) within required timeframes.
- Work collaboratively with families, teams, and external stakeholders to achieve positive outcomes for participants with highly complex support needs.
- Guide and support Support Workers, ensuring they provide care in line with support plans.
- Lead by example—providing hands‑on support to participants and promoting a strong, person‑centered care approach.

About You

- Experience in supporting individuals with disabilities,



particularly those with complex needs.
- A Certificate IV in Aged Care, Disability, or Community Services (or equivalent), along with a current First Aid/CPR certificate.
- Proven leadership skills in managing and supervising teams in SIL homes.
- Training & experience in Manual Handling.
- Experience working with individuals with Intellectual Disability, Epilepsy, Schizophrenia (highly regarded).
- Experience in managing rosters.
- A valid NDIS Worker Screening Check.
- Happy to work in an environment with cats.
