We are seeking dedicated Support Workers to join our Supported Independent Living team in Norton Sands . In this role, you will support participants to live as independently as possible while engaging actively in their community.
Key Responsibilities
- Assist participants with personal care and daily living activities, including meal preparation, household tasks, and medication reminders
- Support participants to access the community, attend appointments, and engage in social and recreational activities
- Follow individualised Behaviour Support Plans and Risk Assessments to ensure safe, high-quality care
- Maintain accurate documentation, including shift notes, incident reports, and data collection
- Collaborate with team leaders to implement and review participant support plans
- Maintain qualified relationships in line with NDIS and organisational standards
- Contribute to positive participant outcomes and team objectives
- Be reliable, accountable, and person-centered in all aspects of care
Qualifications & Experience
- Certificate III in Individual Support, Aged Care, or higher (or willingness to obtain)
- Experience supporting participants with complex diabetes
- Competent in BGL monitoring and recording
- Ability to administer insulin (pen or syringe) safely
- Experience testing and recording ketone levels
- Ability to follow diabetes care plans and protocols
- Experience in mental health and challenging behaviours
- Nursing experience highly regarded
