Exercise Physiologist – Part Time, Ongoing Contract. 72 hours per fortnight.
We are seeking qualified Exercise Physiologists to provide best practice care to patients in the Transition Care Program (TCP), improving physical outcomes and independence for older adults transitioning to community after inpatient stay. This role is flexible, working across both inpatient TCP and community TCP programs within a multidisciplinary team.
Key responsibilities
- Deliver individualized exercise and health promotion interventions for older adults in TCP.
- Collaborate with care coordinators, physiotherapists, occupational therapists and allied health assistants.
- Apply clinical reasoning skills to assess, plan, implement and evaluate exercise programs.
- Assist patients to achieve optimal physical outcomes and independence during transition to community.
Qualifications and skills
- Registered Exercise Physiologist with appropriate qualifications and experience.
- Strong clinical reasoning skills and ability to provide holistic,
patient‑centred interventions.
- Enthusiastic about improving health outcomes for older adults.
- Experience working with older adults in a transitional care or community setting is desirable.
- Ability to work collaboratively in a multidisciplinary team.
