St Patrick's Community Support Centre Limited (or St Pat's as it is affectionately known) is an award-winning not-for-profit homelessness support service and community housing provider. We have been working side-by-side with vulnerable people in the South Metropolitan region of Perth for more than 50 years. Our mission is to end homelessness in our community.

About the Program

Delivered by St Pat’s, the Family Support Program supports families experiencing crisis as a result of homelessness, offering a specialised service and strong referral pathways of support to improve safety and stability within the family unit until sustainable housing can be found.

About the Role

The Assertive Case Worker provides outreach based, client-centred support to families at risk of or experiencing homelessness. The role focuses on building strong relationships with families in complex situations, supporting them to stabilise their housing, strengthen family functioning and access relevant services.

Key Responsibilities will include:

- Provide assertive outreach and intensive case management to families experiencing or at risk of homelessness.
- Conduct comprehensive assessments of housing, financial, safety and wellbeing needs.
- Work to identify and engage the target groups within Housing First frameworks and best practice principles.
- Develop, implement and regularly review client centred,



child centred and strengths‑based case plans in collaboration with families.
- Support families to navigate the housing system, including accessing crisis, transitional and long‑term accommodation options.
- Provide activity‑based work and/or daily living skills support based on an established care plan and risk assessment.
- Assist with applications, referrals, and advocacy across housing, Centrelink, mental health, drug and alcohol, family violence and legal systems.
- Provide support or link in with external supports to address the wellbeing of child/children in the family structure.
- Respond sensitively to the effects of trauma, intergenerational disadvantage, and cultural identity.
- Support parents in improving parenting capacity, routines and household stability.
- Qualifications in relevant areas such as Social Work, Psychology, Community Services or related field.
- Demonstrated experience in case management with families, particularly in the homelessness, housing or child and family services sectors.
- Understanding of the drivers and impacts of homelessness, family violence, poverty and trauma.
- Strong interpersonal and engagement skills, including with clients from Aboriginal and Torres Strait Islander diverse backgrounds.
- Ability to work independently in outreach settings and manage competing demands.
- Sound knowledge of relevant legislation, service systems and referral pathways.
- High level of written and verbal communication, including documentation and advocacy.
- Experienced and competent in the use of Microsoft office and client reporting systems.
