Care Connect invites applications for a Care Manager in our Out of Hospital Care program, based in Bella Vista. This full time position on a 12‑month parental leave cover focuses on assessing, planning and managing services to help clients safely return home from hospital with independence and choice.
You will develop person‑centred care plans, coordinate referrals, and work with clients to ensure the right supports are in place.
